Pdf Summary
"A Matter of Balance" (MOB) is an evidence-based program developed by Boston University's Roybal Center aimed at reducing the fear of falling and increasing activity levels among older adults concerned about falls. Initially clinician-led, high costs limited widespread implementation. The MOB Lay Leader Model offers a low-cost alternative, enabling trained non-clinical leaders, such as health coaches or specialists, to facilitate classes.

The program involves 8 two-hour weekly sessions (in-person) or 9 virtual sessions, with groups of 8-12 participants, typically aged 60 and above, average age 79. Classes include group discussions, problem-solving, assertiveness and exercise training, skill-building, videos, and sharing practical solutions. Participants learn to view falls and fear of falling as controllable, set realistic activity goals, modify environments to reduce risks, and promote strength and balance exercises.

Fear of falling affects 33-50% of older adults, linked to decreased life satisfaction, increased frailty, depression, and lower mobility and social engagement. MOB targets individuals who are concerned about falls, have fallen before, restrict activities due to fear, or want to improve flexibility, balance, and strength.

MaineHealth is the sole licensee and trainer for MOB in its region, providing master trainers who certify lay leader coaches to deliver the program. Comprehensive materials, including manuals, videos, and digital toolkits, support implementation alongside quarterly virtual update meetings. Licensing includes a 3-year term with renewal fees, requiring at least one master trainer per licensed organization.

MOB is cost-effective, with reported healthcare savings of approximately $938 per year per participant and overall reductions in healthcare utilization. The program is available in multiple languages and includes virtual delivery options, enhancing access for diverse older adult populations. Resources and training details are managed through MaineHealth to maintain program fidelity and support ongoing community-based fall prevention efforts.
